How does in home end of life care work?
End of life care (also known as hospice care) involves a team approach to ensuring the care and comfort of a terminally ill patient at home. The end of life care provided is specifically tailored to the needs and wishes of the patient. Support is also provided to the patient’s family members.
The first step in end of life care is developing the patient’s care plan. This involves consulting with the patient (if possible), the patient’s physician, family, clergy, and any other caretakers involved in their care.
The goal of the patient care plan is to provide the patient with the necessary physical and emotional support he or she needs to control symptoms, remain pain-free, and live out their final days with dignity.

What will the end of life care team do for us?
In addition to providing relief and comfort to family caretakers, you can rely on our end of life care staff to:
- Manage the patient’s symptoms and pain.
- Provide other routine care services, such as bathing and incontinence care as needed.
- Provide the necessary medical supplies, medications, and equipments.
- Educate the family on how to meet the patient’s end-of-life care needs.
- Assist the patient with the psychological, emotional and spiritual aspects of dying.
- Provide surviving family and friends with bereavement care and counseling.
- Assist with arrangements for in-patient care if at any time at-home care is no longer an option.

Our in home end of life care services are available 24 hours a day, 7 days a week, including holidays and weekends. We also offer a 24-hour call service to assist families with their end of life care needs and questions outside their regularly scheduled in home care visits.
